Job Description:

This position coordinates all aspects of patient flow throughout the East Market. Coordinates activities between defined areas and provides clinical leadership and expertise to ensure proper patient placement. Facilitates an atmosphere of collegial relationships among nursing personnel and others. Has major responsibility for the implementation of the vision, mission, plans, and standards of the organization and nursing services within their defined areas of responsibility.

Education Qualifications:

    Key Responsibilities:

    • Receives requests for information on medical conditions, medications, diagnostic tests, and prescription refills
    • Will assist with the coordination of patient transfers from referring facilities calling in to the Essentia Health STAT doc line
    • Will process prescription refill requests received from pharmacies and patients, utilizing the Essentia Health primary care refill protocol.
    • Ensures continuous 24/7 coordination of patient transfers and emergent care events. This role is critical for managing time-sensitive cases, including STEMIs, strokes, and other emergent transfers, and its absence creates significant operational and patient safety risks.
    • The Patient Flow Coordinator serves as the central hub for patient movement across the organization and with external facilities. Responsibilities include coordinating interfacility transfers, facilitating emergent care pathways, communicating with providers and nursing leadership, and ensuring appropriate bed placement and resource utilization.
    • This role operates continuously, including nights, weekends, and holidays, and directly supports regulatory compliance, throughput, and patient safety. Delays or gaps in coverage place increased burden on clinical teams and leadership and can negatively impact outcomes for high-acuity patients.

    Licensure/Certification Qualifications:

    Licensure:

    • Current RN license in the state of North Dakota, Minnesota, and/or Wisconsin upon hire.
    • Ability to obtain RN license in all 3 states within 3 months of hire.
