Role Description

Your North Star: Provide and facilitate amazing personalized support to our patients that focuses on frequent engagement, identifying and escalating medical needs, and connecting patients to community-based services to address social drivers of health (SDOH).

We are hiring for a teammate to provide virtual support to our pregnant and postpartum patients. In this role, you will work closely with members of an interdisciplinary care team (e.g., nurses, dietitians, licensed clinical social workers) as the primary point of contact for perinatal patients. Your responsibilities will be to engage and build trust with patients, identify needs, and connect them with the right care at the right time to improve their health. You will build supportive relationships with patients as you work with them over weeks to months. Support may be provided via text, phone, or video calls. This is a full time, salaried position and working hours are: M-F, 10am-7pm ET

In the role, you will:

  • Partner with patients to build trust, identify goals and needs, increase engagement in their healthcare, address barriers (e.g., transportation, housing, food), and drive the execution and adherence of their care plan
  • Connect patients to community-based resources, empowering them to address various psychosocial needs with the support of local services
  • Perform virtual visits in collaboration with our OB providers
  • Proactively outreach to and regularly follow up with patients virtually to check in and make sure they are getting the care and support they need
  • Collaborate with the multidisciplinary care team (e.g., OB providers, RNs, IBCLCs, dietitians, care coordinators, therapists, etc.) to ensure comprehensive and integrated medical and social care for patients throughout their perinatal journey
  • Reduce care gaps (missed appointments, medication management, etc.) through frequent engagements with patients and internal referrals to clinicians as needed
  • Ensure prompt, accurate documentation of all patient interactions and escalate to care team members when necessary
  • Support the development of programs and product by providing user feedback

Requirements

Who you are:

  • 4+ years healthcare experience working in either an inpatient or outpatient OB setting as a medical assistant, nursing assistant, inpatient unit secretary/coordinator, or equivalent
  • Experience working with perinatal patients who have had pregnancy related diseases such as diabetes, hypertension, perinatal loss, etc.
  • Excellent customer service and strong, empathetic communication skills
  • Internet-connected, able to work remotely via video, phone, and text
  • Extensive experience with navigating local community resources and programs
  • Experience providing empathetic, culturally humble care
  • Strong interpersonal, relationship building, and communication skills
  • Strong attention to detail and ability to prioritize across multiple tasks
  • Facile with email, spreadsheets, and willingness to work with new software
  • Flexible and agile thinker who readily embraces change

Bonus points if you

  • Have experience providing virtual care or working in a fast-paced environment such as a high-growth company or startup
  • Have experience using Athena as an EMR
  • Are fluent in Spanish & English

Our salary ranges are based on paying competitively for our company’s size and industry, and are one part of the total compensation package that also includes equity, benefits, and other opportunities at Pomelo Care. In accordance with New York City, Colorado, California, and other applicable laws, Pomelo Care is required to provide a reasonable estimate of the compensation range for this role. Individual pay decisions are ultimately based on a number of factors, including qualifications for the role, experience level, skillset, geography, and balancing internal equity. A reasonable estimate of the current salary range is $35,000-$45,000. We expect most candidates to fall in the middle of the range.
